I am an in-network provider with Pacific Source and Care Oregon (Health Share). I am credentialed with Moda, and will be in-network shortly. If I am out of network with your insurance, I can provide a receipt of services ('superbill') that you can submit for reimbursement.

Any copays or coinsurance will be collected at the beginning of each session. The specific copay amount depends on your plan, and is typically a set fee.

Self Pay

Self-pay is an option if you do not have health insurance or prefer to not use your health insurance for therapy.

Individual sessions are $150 for 55 mins.

Our first appointment will be a thorough formal assessment, with a rate of $195.

Payments and insurance are processed through the telehealth platform, Simple Practice. It is a very stream-lined and secure process.

"Is Emily Crocker, LCSW covered as a preferred provider with my plan or would her services be considered out of network?"

"What is my co-pay or coinsurance cost?"

"Are there limits to the number of outpatient mental health visits covered? If so, how many sessions are covered?"

If out of network: “My therapist is out of network and I will be paying them directly, at $150 per session. How much of the $150 can I expect to be reimbursed through my plan?”

“How many days can I expect my reimbursement to take?”
